Reablement programme

Reablement refers to a limited time to support and provide care as a rehabilitation program that enables individuals to get back to their baseline and to regain their independence in their own home to carry out their daily activities after Hospital admission.
Life changing events such as; old age, disability, illness or an accident may result to finding yourself unable to perform daily tasks as; cooking, bathing, cleaning and other daily routine.
Reablement programme could include making use of mobility equipment, supporting with household chores, medication management and other daily activities.
At My Choice Care Agency, we can support by working with you to develop and achieve a tailored support programme designed to help you to improve your confidence and crucial abilities to regain independence.
Our support workers are committed to offer adequate support with compassion so that you can regain your abilities and look after yourself independently. Our support workers can work with you to become independent in daily activities such as
  • Bathing, washing and other personal care activities
  • Dressing and undressing
  • Preparing meals/drinks
  • Taking your medications
  • Cleaning/laundry/shopping
  • Mobilising around your home
  • Supporting you with exercises (if prescribed)

Person-Centred Care

We provide a wide range of person-centred services to help individuals who choose to live in their own home as safely and securely as possible. Our support workers are trained to offer extensive range of home care services such as the following:
  • Assistance to mobilise/transfers in or out of bed in the morning or evening.
  • Assistance with full shower/strip wash, oral hygiene, towel dry and apply cream to body if required.
  • Getting dressed for the day and undressed to change into sleeping clothes at bedtime.
  • Assistance with shopping and domestic duties.
  • Support with other household chores.
  • Assistance to prepare meals/drinks of choice, serve meals/drinks, clean and tidy up used areas.
  • Help with feeding and drinking if required.
  • Support with medication management.

Shopping and Meal Preparation

My Choice Care always aim to empower and support people receiving care service to manage their daily activities by themselves to maintain their independence as far as possible. However, doing shopping and preparing meals can become challenging as people start experiencing the impact of aging without the right support. Due to old age, other ill health condition and limited mobility, you may need immediate help and support to ensure your safety and comfortable life. As a result, My Choice Care is committed and more than happy to design shopping methodology with you to provide you with the best care possible by making shopping and meal preparation as simple and joyful as it has always been for you. This service will enable you to eat variety of nutritious and balanced foods of your choice tailored to preferences and dietary requirement.
Our proactive, friendly and caring workers are happy to:
  • Assist you with your shopping by discussing your shopping needs, writing a shopping list and your carer do the shopping for you from your preferred Supermarket.
  • Show you the bought items to make sure your shopping has been done according to your needs and they will be stored in the right places as instructed.
  • Support or assist you to prepare healthy and delicious meals.
  • Clear away used dishes and wash them too.
  • Remind you when it’s time to eat or take your medications.

Respite Care/ Sitting Services

Respite care offers essential support and temporary relief to primary caregivers who tirelessly provide assistance to individuals in need of constant care due to medical conditions, disabilities, or age-related issues. This service allows caregivers to take short breaks to attend to their own needs, and have good rest to reenergize while ensuring that their loved ones receive safe and adequate care. Respite care gives the primary caregivers peace of mind and the opportunity to prevent burnout. It plays a vital role in sustaining the well-being of both caregivers and service users by offering much-needed intervals of rest and relaxation amidst the demanding responsibilities of caregiving.
Our proactive, friendly and caring workers are happy to deliver Respite service as the following:
  • Assistance with personal care to get ready for the day and feel comfortable.
  • Assistance with clothing selection.
  • Assistance with meal/drink preparation, serve and clean up.
  • Monitoring diet and eating and medication management.
  • Assistance/Support with medication management.
  • Assistance to arrange medical appointments.
  • Escorting service users to their appointments.
  • Accompanying service users to/assisting with entertaining.
  • Help with reading/participating in hobbies and crafts/buying magazines, papers, and books.

End of life /Palliative Care Support / 24hourly services

Palliative care is referred to people living with terminal illness where there is no
definitive cure possible. Our staff and support workers are experienced and trained
to emotionally focus on providing care service that may relieve service users from
symptoms and stress of the serious illness. Service users will be supported to maintain
inner tranquillity to live a relatively comfortable life as much as possible and peace of
mind for their loved ones. Everybody’s journey is different as such, we respect the
wishes and needs of individuals and support them to live each day accordingly as we
focus on the individual not the illness.

Palliative care/support helps the individual to alleviate symptoms/pain management and offer comfort while supporting them to maintain their dignity and autonomy. My Choice Care team works collaboratively with healthcare professionals, service users and their families to develop personalized care plans that prioritize the individual’s wishes and preferences. By receiving this unique care, the dying individual is able to remain in their own home surrounded by their beloved family members who comfort and support them emotionally.
We are here to help and offer emotional support when you need us and our care services are the following:
  • On call 24/7 for service user’s needs.
  • Flexible and fast scheduling of 24-hourly care support with person centred care, medication management and daily routine tasks.
  • Hourly care for few hours daily or weekly.
  • Thorough care management and family support.

DEMENTIA CARE

People suffering from Dementia may experience health issues as memory issues, behavioural changes such as, asking the same question repeatedly, forgetting something important or doing something inappropriate and impaired decision-making. Sometimes, these individual’s actions and behaviours may be a result of something they have no control over as before. As a result, caring for someone with dementia could be very challenging.
My Choice Care support workers are trained and are able to cultivate the quality of Patience when providing care service to people Suffering from dementia. Our support worker knows that to some extent, the individual’s may display challenging behaviours and behave differently as such, our workers patiently take steps to adjust on how to interact and communicate with the individual. Our workers have excellent communication skills that enables them to engage better with them in their day-to-day activities and to navigate how to effectively respond to the challenging situations that may arise. Our support workers are experts in caring for people suffering with dementia in the following ways:
