Job brief

We are seeking compassionate and dedicated individuals to join our team as Domiciliary Care Coordinator.

Utilising your experience, the Care Coordinator will be responsible for completing team rotas to ensure that the care delivered is of the highest standard. You will work with all internal teams to facilitate patient centred care that enriches the quality of our client’s lives and enables them to live within their own homes whatever their abilities.

Job duties:

  • Liaise closely with lead coordinator or registered manager to manage recruitment needs
  • Process changes to customer care and support plans
  • Ensure there is sufficient cover to deliver excellent care and support to customers
  • Utilise IT systems to allocate care assistants, and appropriately match them to the right customer
  • Rota production and distribution
  • Maintenance of electronic and handwritten records
  • Manage highly confidential and sensitive information

Qualities that are advantageous:

  • Qualified in NVQ Level 2 or NVQ level 3 Health & Social Care desirable
  • Must have previous Care Coordinator experience

The salary for this role is dependent on experience however will likely be in the region of £25,000 – £30,000.

Requirements

  • Previous experience in a similar role preferred but not essential.
  • Compassionate and empathetic nature, with a genuine desire to help others.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Reliable and punctual, with a flexible approach to work hours.
  • Willingness to undergo background checks and relevant training.
  • Valid driver’s license and access to a reliable vehicle preferred but not essential.
